Personal Statement Upload Form

Any attachment larger than 6.5MB (and under 25MB) must be emailed to us at files@omahagirlsrock.com. Please make sure that your camper's name is in the subject line of the email. Any attachment larger than 6.5MB must be sent via mail. Please make sure all contact fields are identical to your regular application.

If submitting by mail, please send to: Omaha Girls Rock 1331 Broadway Burbank, CA 91504 with your camper's name CLEARLY MARKED on the package and the statement itself.
  • No larger than 6.5MB please! File extensions permitted: jpg, gif, png, pdf, mp4, mov, txt, doc, zip, mp3, wav, aiff, avi. If your file is in a different format, please email it to files@omahagirlsrock.com.
    Please be patient while your file uploads. You will receive an email confirmation as soon as the file has been received. If you do not receive this email, please resend your personal statement.
